01 · The Challenge

Healthcare records should move with the patient, not stay behind.The issue isn't a lack of healthcare data. It's a lack of patient ownership.Medical information becomes fragmented every time a patient changes hospitals.

Across many developing regions, medical records are often scattered across hospitals, clinics and laboratories. Patients repeatedly explain their medical history, carry paper files between appointments and undergo tests that have already been performed simply because the right information isn't available when it's needed.

Medical information becomes fragmented across different providers, making continuity of care difficult whenever patients change hospitals, relocate or require emergency treatment.

We believed there was a better way: one where patients remain at the centre of their own healthcare journey, with secure access to their medical information whenever and wherever they need it.

02 · The Platform

The Solution

PH-DVault is a secure health data platform built to help patients organise, manage and securely share their medical records from one central location. Instead of healthcare information remaining locked within individual hospitals or clinics, patients store their records in a secure digital vault and decide exactly what to share, who can access it and how long that access should remain available, through secure links or one-time access codes with configurable expiry periods.

The Platform

Every feature was built around one principle: patient ownership. A Personal Health Vault securely organises records, prescriptions, allergies, lab reports and vaccination history in one place; Selective Data Sharing and Secure Links & Access Codes give healthcare professionals temporary, controlled access; Time-Limited Access and Emergency Access balance strict security with the immediacy care sometimes demands; and separate Patient and Clinician interfaces keep every user working with the tools built for their role.

Security & Trust

Healthcare data is among the most sensitive information a person owns, so PH-DVault applies multiple layers of protection throughout the entire user journey: secure authentication, role-based access, patient-controlled permissions, encrypted data storage and sharing, time-limited links, and a full audit trail of every interaction. Security isn't simply a feature within PH-DVault. It's the foundation the entire platform was built upon.

Create a secure account and set up your personal health profile, giving you one protected place for all your medical information.

Build your medical record

Organise your allergies, medications, vaccination history, laboratory results, medical conditions and supporting health documents inside your secure vault.

Choose what to share

Select only the records relevant to your consultation. Instead of exposing your complete medical history, you decide exactly what information a healthcare professional can access.

Generate secure access

Create either a secure sharing link or a unique access code with configurable expiry periods, giving healthcare professionals temporary access only when it's needed.

Review and provide care

Healthcare professionals securely access the shared information, allowing them to understand the patient's medical history before making informed clinical decisions.

Stay in control

Once the consultation is complete, access automatically expires or can be revoked at any time, ensuring ownership of every medical record always remains with the patient.

06 · The Outcome

PH-DVault demonstrates how thoughtful product strategy and user-centred design can transform the way healthcare information is managed. Instead of fragmented paper records and disconnected healthcare systems, the platform introduces a secure, patient-controlled approach where individuals have greater visibility, ownership and confidence in how their medical information is stored and shared. The project also demonstrates how digital healthcare experiences can balance accessibility, privacy and usability without forcing patients to sacrifice control over their personal data.
